Cluster Headaches

Cluster headache is among the most severe pain conditions in medicine, and its approved treatment set has changed little in a generation. This brief sets out why Zhittya believes FGF-1 is worth investigating in the disease. The case is mechanistic rather than clinical. Cluster headache is now understood as a hypothalamic timing disorder expressed through the trigeminal system, and four independent bodies of published FGF-1 research, developed for entirely unrelated purposes, each land on a node that pathophysiology implicates. There is no human data for FGF-1 in this indication and the brief claims none. What it argues is narrower: that the convergence is strong enough to justify asking the question, and that a molecule administered to more than 450 patients without a drug-related serious adverse event is one that can reasonably be asked about in a population that has run out of options.
